Job summary

Molina Healthcare is seeking a care coordination professional to support LTSS and high-need members. You will collaborate with an interdisciplinary team to ensure high-quality, cost-effective care and progress toward outcomes.

Responsibilities include comprehensive assessments, waiver enrollment/disenrollment, care plan development, monitoring, and coordination of services across behavioral health and community resources. Travel 25-40% may be required.

Qualifications

• 2+ years health care experience with LTSS and care management.
• Valid driver’s license and reliable transportation.
• Ability to work independently and manage multiple priorities.
• Bachelor’s degree may be required in some states.

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person home visits.
• Develop and implement care plans and waiver service plans with the team and member.
• Monitor care plans, document interventions, and adjust as needed.
• Coordinate waiver enrollment/disenrollment and ICT meetings.
• Promote integrated LTSS with behavioral health and community resources.
