Traditional Home Care in Reading, PA is seeking a Care Coordinator for a salaried, weekly-paid role with a strong bonus program and comprehensive benefits. You’ll monitor and support daily service activities, ensuring quality care and compliance with regulations while coordinating with clients and care teams.

As the Care Coordinator, you will recruit, orient, and support caregivers, manage client caseloads, and balance EVV/payroll duties.

Qualifications

• Previous home health care or home care experience highly desirable
• High School or GED required. Associate’s Degree preferred
• Staffing experience preferred
• Excellent verbal/written communication, time management and organizational skills
• Ability to effectively multi-task
• Effective problem solving skills
• Strong presentation and training skills
• Strong customer service skills

Responsibilities

• Provide superior customer service and quality home care
• Build lasting relationships with clients, referral sources, payers, and community organizations
• Coordinate client services and manage scheduling issues
• Manage and grow your client caseload
• Develop strong, communicative relationships with the team
• Interview, hire, and orient reliable and compassionate caregivers
• Function as primary support for daily EVV and payroll functions
