Sea Mar Community Health Centers in Washington state is hiring a Full-time Care Coordinator I or II for our Puyallup Medical Clinic. The role focuses on assessing gaps in care for patients with chronic conditions and mental health needs, and developing plans with the care team during daily huddles to improve patient-centered care.

Hourly Rate: $21.88 - $22.63
Full‑time Care Coordinator position available for our Puyallup Medical Clinic. The Care Coordinator is responsible for being part of a clinical care team and enhancing quality and patient‑centered care. This is accomplished by assessing gaps in care for patients with chronic conditions and/or mental health needs and creating a plan with the clinical care team during daily huddles. The coordinator will assist patients with medication management, access to insurance, and help identify any other preventive health needs. The coordinator will also assist patients with ongoing self‑management goal setting utilizing Motivational Interviewing skills. Strong computer skills are necessary to track patient adherence with their plan of care in electronic charts. The coordinator must also facilitate team meetings, so organization and effective communication skills are needed.
