Job summary

A community health organization in Kent is seeking a Care Coordinator to support patients with chronic conditions and behavioral health needs. In this role, you will participate in daily huddles, identify patient needs, and coordinate care transitions. Candidates must be LPN or hold a BA/BS/BSW in a health-related field along with strong communication and organizational skills. Bilingual skills in English and Spanish are strongly preferred, along with a compassionate approach to patient care and a valid WA driver's license.

Qualifications
