  • Track adherence and report outcomes to PCP.
  • Lead monthly Health Home meetings.
  • Collaborate to improve Patient-Centered Medical Home processes.
  • Review records for quality indicators.
  • Generate reports to identify improvement areas.
  • Other duties as assigned.

Position Summary

Full-time Care Coordinator II position at Lynnwood Medical Clinic. Responsible for coordination of care for patients with chronic conditions and behavioral health needs at point of care. Works with Clinical Care Team, attends daily huddles, identifies patient needs per protocols, provides point-of-care services, and enhances quality and patient-centered care.

Hourly Rate: $22.63

Duties and Responsibilities
  • Participate in morning huddles to anticipate patient clinical, social and behavioral health needs.
  • Work with care team to identify gaps in care and resolve them using process improvement strategies.
  • Provide brief interventions at point of care to assist patients with chronic illness management, address social needs and link to behavioral health.
  • Advocate for patient services with community, social service and medical providers.
  • Coordinate care transitions for patients discharged from emergency or hospital/long‑term care facilities.
  • Track patient adherence to plan of care in electronic or paper charts and communicate outcomes to primary care provider.
  • Serve as point person within clinic care team for chronic disease management and improvement activities to improve clinical quality measures.
  • Organize and facilitate monthly Health Home meetings with Clinic Operations Team/Manager.
  • Collaborate with clinical care team to improve Patient‑Centered Medical Home processes and document performance.
  • Review medical records for quality and utilization indicators per Quality Improvement Plan.
  • Generate reports for care teams to identify improvement areas and monitor sustainability of each quality measure.
  • Other duties as assigned.

  • Knowledge of the Patient‑Centered Medical Home model and motivational interviewing skills a plus.
  • Knowledge of evidence‑based standards of care for chronic conditions and behavioral health issues.
  • Proficiency in Microsoft Word, Excel, PowerPoint, Outlook.
  • Ability to utilize and document relevant patient information in Electronic Health Record.
  • Knowledge of community resources.
  • Work in a fast‑paced community health care setting.
  • Analytical and problem‑solving in multidisciplinary team and independently.
  • Deal effectively with difficult people and situations.
  • Communicate effectively with diverse communities.
  • Time management and task prioritization.
  • Analyze patient care data.
  • Identify client learning needs and assess knowledge, skill level and readiness for learning.

  • Bilingual in Spanish strongly preferred; other language skills considered.
Education, Certifications and Licensure
  • LPN with ambulatory care experience or BA/BS/BSW in health‑related field with one year community health experience, or 4 years equivalent experience. LPN license not required.
  • Must obtain Basic Life Support (BLS) CPR within 90 days of hire and maintain it.
  • NCQA Certification a plus.
  • Valid Washington State Driver’s License and proof of liability insurance.
