Job summary

Traditional Home Health Care in Wilkes-Barre, PA, is seeking a Care Coordinator to monitor and support daily services for clients in accordance with state regulations and company policy. The role focuses on coordinating services, scheduling, and ensuring quality care in the home setting.

The candidate will work with non-medical staff to deliver ADL assistance, build relationships with clients and referral sources, and ensure smooth EVV/payroll operations.

Qualifications

• Excellent verbal/written communication, time management and organizational skills.
• Ability to effectively multi-task.
• Strong presentation and training skills.
• Staffing experience preferred.

Responsibilities

• Provide superior customer service and quality home care.
• Build lasting relationships with clients, referral sources, payers, and community organizations.
• Coordinate client services and manage scheduling issues.
• Manage and grow your client caseload.
• Develop strong, communicative relationships with the team.
• Interview, hire, and orient reliable and compassionate caregivers.
• Function as primary support for daily EVV and payroll functions.
