Traditional Home Health Care in Reading, PA, is seeking a Care Coordinator to oversee daily services for clients in accordance with state regulations and company policy. The role supports their non-medical division and helps the team deliver high-quality home care.

The position is full-time and salaried, with a generous benefits package, including Health, Dental, Vision, 401K, disability coverage, and paid time off from start of service.

Qualifications

• Previous home health care or home care experience highly desirable.
• High School or GED required; Associate’s Degree preferred.
• Staffing experience preferred.
• Excellent verbal/written communication, time management and organizational skills.
• Ability to effectively multi-task.
• Effective problem solving skills.
• Strong presentation and training skills.
• Strong customer service skills.

Responsabilités

• Provide superior customer service and quality home care.
• Build lasting relationships with clients, referral sources, payers, and community organizations.
• Coordinate client services and manage scheduling issues.
• Manage and grow your client caseload.
• Develop strong, communicative relationships with the team.
• Interview, hire, and orient reliable and compassionate caregivers.
• Function as primary support for daily EVV and payroll functions.
