ECM Lead Care Coordinator – Full Time position located in Youth and Family Programs, Garden Grove, CA 92841. Salary $27.00 - $30.00 hourly. Education level: 4 Year Degree. Job category: Nonprofit - Social Services. This role supports Enhanced Care Management (ECM) procedures, works with a multi-disciplinary team, and aims to build sustainable support for residents.

What You Will Accomplish in This Role

The ECM Lead Care Coordinator will deliver intensive, in-person care management services to vulnerable populations, including individuals and families experiencing homelessness, high utilizers, children and youth involved in child welfare, and tenants in CAP OC housing units. This role addresses clinical and non-clinical needs as part of the member’s multi-disciplinary care team to coordinate all aspects of Enhanced Care Management (ECM).

Salary: $27.00 - $30.00 per hour. Preferred: Bilingual in English/Spanish (written and oral). Regular travel throughout Orange County required. Valid driver’s license, reliable personal transportation, and active auto insurance required. Hybrid schedule: time split between Office, Field, and Remote work; exact split may fluctuate and is subject to change based on program needs.

Roles and Responsibilities

• Comprehensive Case Management: Conduct client screenings and assessments; develop, implement, and monitor individualized care/service plans; provide crisis intervention; connect clients to health, mental health, employment, housing, and other resources (including CalAIM community supports and transportation).
• ECM Services Oversight: Supervise the provision of ECM services and the development/implementation of care plans.
• Documentation: Maintain accurate records of service objectives, outcomes, and other services per guidelines.
• Interdisciplinary Communication: Facilitate clear communication among care team members regarding care plans.
• Provider Coordination: Collaborate with medical, behavioral health, specialists, and housing navigators.
• Client Accompaniment: Accompany clients to office visits as needed.
• Service Coordination: Ensure a seamless client experience and avoid service duplication.
• Approach Utilization: Utilize motivational interviewing, trauma-informed care, and harm-reduction approaches.
• Treatment Monitoring: Monitor adherence to treatment plans, including medications.
• Health Promotion: Provide health promotion and self-management training.
• Reporting: Complete and submit program reports as scheduled or needed.
• Member Engagement: Engage with eligible members.
• Additional Duties: Perform other duties as assigned.

• Medical Terminology: Proficient in medical terminology for communication with healthcare providers.
• Health Insurance: Knowledge of health insurance plans (Medicare, Medicaid, private) and claims processes.
• Comprehensive Care Plans/Service Plans: Ability to create tailored care plans.
• Needs Assessments: Experience conducting thorough assessments of medical, social, and psychological needs.
• Case Management & Housing Services: Expertise in case management and housing services related to poverty issues.
• Coordination with Healthcare Providers: Experience coordinating with providers to cover all aspects of member care.
• Assessment Tools: Proficient in various assessment tools and methodologies.
• Communication Skills: Excellent verbal and written communication with members, families, and providers.

Education and Experience

• Bachelor’s degree in Nursing, Social Work, Public Health, Healthcare Administration, or related field (Master’s degree preferred).
• Advanced certifications in care management (CCM or CPHQ) a plus.
• 3-5 years of experience in care coordination, case management, or related healthcare role; proven experience with homelessness or chronic conditions.

Travel

Possess a valid California Driver’s License with driving record meeting minimum CAP OC standards, proof of vehicle insurance, access to a vehicle, and willingness to drive/travel as required. Travel is local to Orange County; possible day travel to LA County/Inland Empire. Personal vehicle required. Evening, out-of-area, and overnight travel possible.

Physical Demands

Typical physical demands include bending, stooping, reaching, lifting up to 20 pounds, prolonged computer work, and exposure to varying weather conditions when working outdoors. May require outdoor work and varied temperatures.
