The ECM Lead Care Coordinator will deliver intensive, in-person care management services to vulnerable populations, including individuals and families experiencing homelessness, high utilizers, children and youth involved in child welfare, and tenants in CAP OC housing units. This role addresses clinical and non-clinical needs as part of the member’s multi-disciplinary care team to coordinate all aspects of Enhanced Care Management (ECM).
Salary: $27.00 - $30.00 per hour. Preferred: Bilingual in English/Spanish (written and oral). Regular travel throughout Orange County required. Valid driver’s license, reliable personal transportation, and active auto insurance required. Hybrid schedule: time split between Office, Field, and Remote work; exact split may fluctuate and is subject to change based on program needs.
Possess a valid California Driver’s License with driving record meeting minimum CAP OC standards, proof of vehicle insurance, access to a vehicle, and willingness to drive/travel as required. Travel is local to Orange County; possible day travel to LA County/Inland Empire. Personal vehicle required. Evening, out-of-area, and overnight travel possible.
Typical physical demands include bending, stooping, reaching, lifting up to 20 pounds, prolonged computer work, and exposure to varying weather conditions when working outdoors. May require outdoor work and varied temperatures.
