Growth Path: Lead Care Coordinator
About NeuralCare

NeuralCare is building an AI-powered dementia care platform designed to help families manage the day-to-day complexity of caring for someone with dementia.

Our platform combines AI agents with experienced human coordinators to help caregivers manage appointments, medications, benefits, care resources, research, follow-ups, and the hundreds of small tasks that make dementia caregiving overwhelming.

We are looking for a Care Coordinator who can work directly with families today and help us build the care coordination model of tomorrow.

This is not a traditional call-center or scheduling role. You will become a subject matter expert inside the company, working closely with caregivers, product teams, engineers, and AI teams. Over time, we expect the right person to help train and lead a team of NeuralCare coordinators.

What You Will Do
Support patients and family caregivers

You will work directly with families caring for people with dementia and help them solve real problems, including:

  • Understanding care needs and helping families determine appropriate next steps
  • Finding physicians, specialists, home care, adult day programs, respite care, transportation, support groups, and community resources
  • Helping caregivers understand and navigate Medicare, Medicaid, VA benefits, and other available programs
  • Supporting appointment scheduling, preparation, follow-up, and care coordination
  • Researching services and resources when the answer is not obvious
  • Following open issues through completion rather than simply giving families a phone number or website
  • Recognizing when a situation requires escalation to a clinician, emergency service, social worker, or another qualified professional
  • Building trusted, ongoing relationships with caregivers who may be under significant emotional and practical stress
Help build NeuralCare’s product

Our coordinators are also subject matter experts for the product team.

You will:

  • Identify recurring caregiver problems that NeuralCare should solve
  • Help define workflows, features, and product priorities based on real caregiver needs
  • Review new features before they are released and participate in user acceptance testing
  • Create realistic dementia-care scenarios, workflows, and edge cases for product development
  • Identify gaps where technology may give an incomplete, unsafe, or impractical answer
  • Help define when the AI should act independently, ask additional questions, involve a coordinator, or elevate to a healthcare professional
Help train and evaluate NeuralCare’s AI agents

NeuralCare is developing AI agents that communicate with caregivers, patients, healthcare organizations, and community resources through messaging and voice.

You will help us make those agents useful, safe, and human.

Responsibilities will include:

  • Reviewing conversations between AI agents and caregivers or patients
  • Evaluating whether responses are accurate, empathetic, clear, and appropriate
  • Helping create conversation guidelines, escalation rules, and response standards
  • Developing test scenarios based on situations you have seen in real-world care
  • Testing agents that call physician offices, pharmacies, community organizations, senior services, and other external resources
  • Identifying situations where an agent should stop and involve a human
  • Providing structured feedback to product and engineering teams so agent performance improves
What We Are Looking For
Required
  • 4+ years of relevant experience in healthcare, aging services, social services, care management, case management, patient navigation, caregiving, or a closely related field
  • Strong working knowledge of the U.S. healthcare system
  • Practical understanding of Medicare, including Original Medicare, Medicare Advantage, Part D, and common coverage/navigation issues
  • Working understanding of Medicaid, including its role in long-term services and supports and the fact that eligibility and programs differ by state
  • Familiarity with VA healthcare and benefits available to veterans and their families, or demonstrated ability to become proficient quickly
  • Experience helping patients, seniors, caregivers, or families solve problems across multiple healthcare and community organizations
  • Excellent verbal communication skills and comfort speaking with families by phone or video
  • Strong written communication and documentation skills
  • Ability to research unfamiliar questions, separate reliable information from poor information, and turn research into a practical next step
  • Strong follow-through. You should enjoy taking ownership of an unresolved problem and getting it to closure.
  • Comfort using technology and learning new software quickly
Strongly Preferred

One or more of the following would make you particularly well suited for this role:

  • Direct experience supporting people living with Alzheimer’s disease or other dementias
  • Professional experience as a social worker, care manager, geriatric care manager, patient navigator, community health worker, nurse, or senior-care professional
  • Personal experience as a family caregiver combined with relevant professional experience
  • Dementia-specific training or certification, such as Certified Dementia Practitioner (CDP) or equivalent
  • Background in gerontology, social work, nursing, psychology, public health, occupational therapy, or a related discipline
  • Experience with Area Agencies on Aging, senior-service organizations, memory clinics, home health, hospice, adult day programs, skilled nursing facilities, assisted living, or aging-in-place services
  • Experience navigating Medicaid waiver programs, VA Aid and Attendance, Medicare Advantage supplemental benefits, or community-based aging programs

A bachelor’s or master’s degree in social work, nursing, gerontology, public health, psychology, or a related field is preferred, but substantial relevant experience can be equally valuable.

Families dealing with dementia need someone who will listen carefully without making every interaction feel heavy. You should be warm, patient, optimistic, and energetic.

A problem solver.

If a caregiver says, “My mother cannot be left alone anymore and I don’t know what to do,” your instinct should be to understand the situation, identify the options, make the calls, and help build a path forward.

Comfortable with ambiguity.

Healthcare rarely produces neat answers. Benefits differ by state, organizations have different rules, and families frequently do not know what services exist. You should be comfortable researching and working through those situations.

Curious about technology and AI.

You do not need to be technical or know how to build an AI model. You do need to be interested in teaching technology how good care coordination should work.

Able to become a leader.

We are specifically looking for someone who could grow into leading NeuralCare’s coordinator function. That means setting standards, coaching other coordinators, reviewing quality, improving workflows, and representing the coordinator perspective in product decisions.

During your first several months, you would learn the NeuralCare platform, work directly with caregivers, and become deeply familiar with the problems they bring to us.

As the organization grows, we expect this role to expand into areas such as:

  • Training and mentoring new coordinators
  • Developing coordinator playbooks and quality standards
  • Reviewing complex cases and escalations
  • Establishing service-level and follow-up standards
  • Measuring caregiver satisfaction and coordination quality
  • Helping define which activities should remain human-led and which can safely move to AI agents
  • Leading testing and quality assurance for patient-facing and caregiver-facing AI agents
  • Representing care operations in product roadmap discussions

For the right person, this role can grow into Lead Care Coordinator / Care Operations Lead as NeuralCare scales.
