Youth Villages in the United States seeks a Care Coordinator to deliver high-quality wraparound crisis care for youth up to 18. You will develop and monitor Individual Service Plans, coordinate family team meetings, and link families to community resources for ongoing stability.

Qualifications

• Bachelor’s degree in a social services discipline is required.
• Experience navigating child-serving systems is required.
• Experience working with children/youth and families with significant behavioral health needs is required.
• Ability to provide a strength-based, client-centered approach to service delivery is required.
• Nonprofit or community-based experience is preferred.

Responsibilities

• Provides wraparound crisis care in a family-driven, youth-guided model.
• Develops and monitors Individual Service Plans with families.
• Coordinates family team meetings and links to community resources.
• Maintains consistent communication with families and providers.
• Facilitates transitions and discharge planning as needed.
• Completes documentation in an EMR system in a timely manner.
• Participates in weekly meetings with family/youth and for professional development.
