The Care Coordinator is a key member of Triangle Community Center’s (TCC) care coordination team, providing direct support to LGBTQIA+ individuals navigating mental health, housing, healthcare, and basic needs systems. This role is grounded in relationship building, advocacy, and practical problem solving - ensuring that each client is met with dignity, affirmation, and a clear path forward.
The Care Coordinator works closely with clients, staff, and community partners to deliver responsive, client-centered, and trauma-informed care. This position plays a critical role in advancing TCC’s mission to ensure LGBTQIA+ individuals have access to the resources and support they need to thrive.
You approach this work with a deep respect for the dignity, autonomy, and lived experiences of the people you serve. You understand that care coordination is not simply about connecting clients to resources; it is about building trust, navigating systems that were not designed with everyone in mind, and showing up consistently in ways that are affirming, reliable, and human.
