Qualifications

• 1-3+ years of experience in case management, care coordination, or social services.

Responsibilities

• Provide individualized care coordination, including intake, assessment, goal setting, and ongoing follow-up.
• Advocate for equitable access to resources for LGBTQIA+ individuals.
• Maintain accurate and timely case documentation.
