Job summary

Howard Center seeks a Care Coordinator for the Bridge team in Vermont. The role supports families and clients, conducts meetings, initiates contact, attends home visits and team meetings, and coordinates a client's team of supports.

The Bridge team serves children up to 22 with intellectual disability and/or autism, focusing on care coordination, case management, and school meetings.

Qualifications

  • QDDP - Qualified Developmental Disability Professional.
  • On the Roster of Non-Licensed & Noncertified Psychotherapists (See Licensing Requirement Notice below).
  • 2-3 years' experience in human services field with an emphasis on children with special needs.
  • Proven team leadership ability.
  • Valid driver's License, and use of a personal vehicle.

Responsibilities

  • Provide services to children and young adults with ID and/or autism spectrum disorder.
  • Assist families in identifying and securing needed supports and attending school meetings.
  • Maintain timely, accurate documentation for services, needs assessments and referrals.

Job description

Care Coordinators support families and clients by conducting regular meetings, initiating contact, attending home visits and team meetings, and managing a client's team of support.

Care Coordinators are responsible for eligibility determination, assessment of need, plan development to address a child's needs and coordination of multiple services. This position is within the Bridge team which serves children and young adults, up to the age of 22, who have intellectual disability and/or autism spectrum disorder.

Bridge

serves children, up to the age of 22, with an intellectual disability and/or autism spectrum disorder. Our main service is Care Coordination. Supports include case management/service coordination, meeting with essential providers, family support, and attending school meetings.

Responsibilities
  • Provide a broad spectrum of services to children and young adults with an intellectual disability and/or autism spectrum disorder.
  • Per individual design, assist families in identifying, procuring and sustaining needed supports to ensure their ability to continue living with clients being served.This could be accessing community resources, attending school meetings, supporting with transition services, etc.
  • Timely and accurate documentation for service rendered, needs assessments, referrals, etc.

Licensing Requirement Notice

Vermont regulation mandates that if you do not hold a valid clinical license then you must added to the Roster of Non licensed & Noncertified Psychotherapists. Note this process may take 3-5 days for in-state applications and up to several weeks for out-of-state applications. Instructions can be found on the website for the Vermont Office of Professional Regulation
