RiseWell Community Services is seeking a dedicated Bilingual (Spanish speaking) Care Coordinator to join our Care Coordination Department. This remote role requires field visits across Long Island (Nassau & Suffolk County).

Salary: 38k-40k Annually
In this role, you will provide care coordination and health promotion to individuals with severe mental illness and chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits, and unnecessary care. You will oversee and coordinate access to all services (behavioral, medical health, and social service supports) to assist individuals in staying healthy. You will complete all required documentation according to standards and no later than 48 hours of service delivery.
QUALIFICATIONS:
Minimum of a Bachelor’s degree or a CASAC with at least 2 years of experience serving people with mental illness, developmental disabilities, or alcoholism or substance abuse. A clean, valid NYS Driver’s License is required. Fingerprinting, criminal record check, and approval from NYS Office of Mental Health are also required.
