Job summary

Sweetser is seeking a Care Coordinator to support individuals in the Community Behavioral Health Clinic (CCBHC) in Maine. You will coordinate care, facilitate collaboration among providers, and assist with intake and ongoing monitoring of needs.

You'll document all contacts in an Electronic Health Record, complete intake/discharge surveys, and help clients access benefits and services. A Bachelor's degree is required and 1–2 years of mental health experience is preferred.
