Job summary

Immediate need for a Care Coordinator in Wichita, KS. This is a 06+ Months Contract onsite with long-term potential. Pay range is $22.5-$23/hour with benefits including health insurance, 401(k) and sick leave.

Key Responsibilities:

• Perform member outreach telephonically or through home visits to engage members and discuss care/service plans.
• Coordinate care activities based on the care plan/service plan and work with healthcare providers and community partners.
• Serve as a liaison alongside care managers and providers to ensure proper coordination of care.
• Document and maintain member records in accordance with state and regulatory requirements.
• Provide education on benefits and resources available to members.
• Assist with training and development needs as necessary.
• Comply with all policies and standards, ensuring adherence to contractual requirements and regulatory guidelines.

Key Requirements and Technology Experience:

• 2 - 4 years of related experience in healthcare record review, auditing, medical record abstraction, care management documentation, HEDIS, LTSS, Medicaid, or HCBS.
• High School diploma or GED required.
• Bachelor’s degree in nursing, social sciences, social work, or related field preferred.
• One (1) year of supervised clinical experience in a human-services field preferred.
• Must reside in Illinois, Florida, or Arkansas depending on the specific care plan.
