Immediate need for a Care Coordinator in Wichita, KS. This is a 06+ Months Contract onsite with long-term potential. Pay range is $22.5-$23/hour with benefits including health insurance, 401(k) and sick leave.

Responsibilities include member outreach, care coordination, documentation, and regulatory compliance. Required 2–4 years in healthcare-related fields, HS diploma, and preferred Bachelor's degree in a related field. Residency varies with plan.

Qualifications

• 2–4 years of healthcare experience in record review, auditing, or care management documentation.
• High School diploma or GED; bachelor's degree preferred.
• Must reside in Illinois, Florida, or Arkansas depending on the care plan.

Responsabilités

• Perform member outreach telephonically or through home visits to discuss care plans.
• Coordinate care activities with providers and community partners.
• Serve as liaison with care managers and providers to ensure care coordination.
• Document and maintain member records per state and regulatory requirements.
• Provide education on benefits and resources to members.
• Assist with training and development as necessary.
• Comply with policies and regulatory guidelines.
