Job Description:
The Care Coordinator is a non- clinical, patient-focused role that helps successfully manage the comprehensive care of patients. This position provides customer service, proactive outreach to patients, and administrative support to clinicians and care teams. The Care Coordinator role is patient facing via phone almost all day. Care Coordinator’s are responsible for managing inbound and outbound calls to schedule appointments, utilizing analytics to help close gaps in care, supporting patients to meet their goals, coordinating resources to help patients overcome socioeconomic barriers, and resolving patient issues when possible. This includes receiving, prioritizing, documenting, and actively resolving caregiver requests. This position reports to a Care Coordination Supervisor and works collaboratively with the Care Coordination Manager, Operations Transformation, Network Management, Care Management, Providers, and various members of clinic staff.Essential Functions

Affiliate Pediatric Care Coordinator The position primarily revolves around outreach for patient scheduling, but candidate may also assist with preventative care outreach campaigns, conduct virtual meetings with physicians, and complete social needs resourcing referrals as the clinic integrates services over time. Must also assist with various tasks using a variety of electronic health records systems.
Affiliate Quality Care Coordinator:Proactive role in supporting our Affiliated clinics with Care Gap Closure and High Value Scheduling, for their panel of patients. Initiates & leads Huddles at the appropriate cadence with Clinic Point of Contacts. Works closely with our Network Management Team.
Strategic Support Team: Care Coordination team without clinic assignments that is dedicated to internal and external stakeholder requests. The Strategic Support Team's primary focus is to connect patients to preventative care gap scheduling and assist patients in getting established with primary care via outbound call campaigns. Additionally, the team provides comprehensive department coverage, ensuring seamless coordination and support across various healthcare services.
IMG Quality Care Coordinator - Proactive role in supporting IMG clinics with Care Gap Closure, High Value Scheduling, and Admission, Discharge & Transfer data for specifically assigned IMG Clinician panels. Initiates and leads huddles at the appropriate cadence with assigned Clinicians as well as SNCCs, Care Managers, and MAs as appropriate.
