Job summary

Griswold Home Care NJSCFL is seeking a Care Coordinator for our Southwest Florida office. This in-office role blends customer service with case management, scheduling, and caregiver coordination in a family-oriented team.

You will engage with clients, families, and caregivers, conduct telephone intakes and home visits, and ensure high-quality, compassionate care while maintaining accurate records and compliance.

Qualifications

  • Fluent in English and Spanish, able to communicate with clients, families, and caregivers.
  • Excellent communication with clients, families, and caregivers in professional settings.

Responsibilities

  • Coordinate scheduling with Office Manager and handle administrative tasks.
  • Conduct telephone intakes and home visits with clients using company vehicle.
  • Perform follow-up visits to ensure satisfaction and quality of care.
  • Manage caregiver placements, replacements, and substitutions.
  • Resolve concerns with empathy and professionalism.
  • Maintain accurate client and caregiver files for compliance.
  • Screen, interview, and match caregivers to client needs.

As a Care Coordinator, you’ll be at the heart of our operations in a role that blends meaningful customer service, relationship-building, scheduling, and case management. You’ll help ensure our clients receive compassionate, high-quality care while working alongside a team that feels like family. This is an in-office position full of variety — from engaging phone conversations and team collaboration to occasional community visits — in a fun, collaborative setting where your contributions are valued and celebrated daily.

What You’ll Do:
  • Partner with our Office Manager on scheduling, marketing, recruiting, and key administrative tasks that keep our office running smoothly.
  • Conduct thoughtful telephone intakes and initial home visits (using our company vehicle) with new or prospective clients — explaining our services and helping secure service agreements.
  • Perform follow-up visits with existing clients to ensure their satisfaction and the highest quality of care.
  • Manage all scheduling needs, including caregiver placements, replacements, and substitutions.
  • Resolve client, family, and caregiver concerns with empathy, professionalism, and creative problem-solving.
  • Maintain accurate, up-to-date client and caregiver files to support compliance with internal, state, and federal standards.
  • Screen, interview, and thoughtfully match caregivers to clients’ unique needs.
