Job summary

Sentara Health Plans is hiring a Care Coordinator for the Central region (Farmville, Kenbridge), VA. The role involves case management for patients and families, collaborating with a multidisciplinary team, and assisting with medical and environmental challenges through to end-of-life care.

Standard hours are 8am-5pm EST, full-time (40 hours/week). Travel up to 90 minutes one way may be required. Base pay ranges from $24.91 to $40.07 hourly, with additional compensation possible.

• Assist adult patients and families with personal and environmental difficulties related to medical conditions
• Experience in mental health is preferred

Location: Applicants must be in- Central (Farmville, Kenbridge), VA. Travel may include up to 90 minutes one way.

Job Responsibilities

Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.

Must possess a degree in Health & Human Services or one of the following related Fields:

Certification/Licensure

Experience

• Long Term Care- 1 year (required)
• Health Plan- 1 year; Medicaid- 1 year (preferred)
• At least one year of mental health experience (preferred)
• 1 year of experience with Children/Adolescents (required)

Compensation

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $24.91 - $40.07 hourly. Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
