Job summary

Qualifications

  • Three years of experience with children and families in behavioral health or related field.
  • Hi-Fidelity Wraparound knowledge preferred; certification provided at time of employment.
  • Two years in coordinated supportive services or care coordination role preferred.
  • Experience with autism spectrum disorders and developmental disabilities preferred.
  • Experience in family systems, community resources, case management, or child development.
  • Ability to bill Medicaid and maintain confidentiality.

Responsibilities

  • Create, plan, organize, lead, and facilitate child and family team meetings.
  • Provide care coordination in families' homes, communities, or preferred locations.
  • Partner with youth, families, natural supports, and service providers to identify needs and services.
  • Support youth at home, in school, and in their communities.
  • Identify needs, goals, and action steps to meet family goals.
  • Ensure youth and families have voice and choice in coordinated care and services.
  • Maintain documentation for OhioRISE benefits.
  • Meet OHIORISE quality and fidelity requirements.
  • Maintain knowledge of community resources.
  • Attend trainings and supervision, including travel.
  • Complete trainings and maintain certifications.
  • Follow confidentiality and HIPAA requirements.
  • Use EHR systems for client documentation.
  • Travel within assigned service area.
  • Perform other duties as assigned.

Job description

  • Create, plan, organize, lead, and facilitate child and family team meetings
  • Provide care coordination in families' homes, communities, or preferred locations
  • Partner with youth, families, natural supports, and service providers to identify needs and services
  • Support youth at home, in school, and in their communities
  • Identify needs, goals, and action steps to meet family goals
  • Ensure youth and families have voice and choice in coordinated care and services
  • Maintain documentation to access and maintain OhioRISE benefits
  • Meet ISBH OhioRISE quality and fidelity requirements
  • Maintain knowledge of community resources
  • Attend required trainings and supervision, including travel
  • Complete assigned trainings and maintain required certifications
  • Follow confidentiality, professionalism, incident-reporting, HIPAA, mandated-reporting, and ethical requirements
  • Use databases and Electronic Health Record systems for client documentation
  • Travel within the assigned service area
  • Perform other assigned duties
Requirements
  • Three years of experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or related public sector human services/behavioral healthcare field with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher
  • Knowledge and experience in Hi-Fidelity Wraparound preferred; certification provided at time of employment
  • Two years of experience in a coordinated supportive services or care coordination role preferred
  • Experience working with people with autism spectrum disorders and developmental disabilities preferred
  • Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
  • Qualifications and experience working with children and families experiencing serious emotional disturbance, trauma, co-occurring behavioral health disorders, and engagement with child-serving systems
  • Effective verbal and written communication
  • Ability to use a personal or company vehicle in homes, schools, and community settings
  • Ability to maintain confidential information
  • Ability to adhere to safety rules, regulations, and requirements
  • Ability to bill Medicaid without restrictions
  • Valid driver's license and ability to operate a motor vehicle
  • Appropriate auto insurance coverage
  • Ability to manage deadlines in a fast-paced, high-volume environment
  • Ability to operate in an internet-based, automated office environment
  • Ability to conduct on-camera video calls and meetings
  • Minimum 100 Mbps high-speed internet connection
  • Prolonged sitting and computer work
  • Ability to lift up to 15 pounds

Demonstrates expertise in care coordination and support for children and families, particularly in behavioral health and developmental disabilities. Proficient in maintaining documentation, adhering to confidentiality standards, and utilizing electronic health record systems.
