Job summary

360 Behavioral Health is seeking a Care Coordinator in Van Nuys, CA to lead Direct Support Professionals, supervise care delivery, and ensure families receive high-quality, compassionate support.

You will collaborate with Regional Centers and behavioral agencies, conduct staff meetings and professional development, and provide clinical guidance while maintaining accurate documentation.

Qualifications

• 1+ year clinical experience working with individuals with diverse needs.
• Supervisory experience (preferred).
• Strong leadership, organization, and communication skills.
• Ability to work independently and collaborate with a multidisciplinary team.
• Comfortable in diverse home environments and culturally responsive.

You’ll oversee Direct Support Professionals, collaborate with families and Regional Centers, and ensure every client receives high-quality, compassionate care.

• Competitive compensation: $23.00–$25.00/hour
• Required Availability- Monday-Friday 8:00am-5:00pm evenings and weekends as needed.

Your Role:

What We’re Looking For:

Education & Certifications:

• High School Diploma/GED required; pursuing bachelor’s degree preferred
• Must pass background check/Live Scan
• TB clearance, CPR/First Aid, and Crisis Prevention Certification
