Job summary

360 Behavioral Health, Inc. is seeking a Care Coordinator to lead Direct Support Professionals, collaborate with families and Regional Centers, and ensure high-quality, compassionate care in a fast-paced environment.

This on-site role in Van Nuys, CA offers $23–$25 per hour, Monday–Friday 8:00am–5:00pm with evenings and weekends as needed, 7 paid holidays, PTO, and comprehensive health, vision, and dental benefits.

Qualifications

• 1+ year clinical experience working with individuals with diverse needs.
• Supervisory experience preferred.
• Strong leadership, organization and communication skills.
• Ability to work independently and collaborate with a multidisciplinary team.

Responsibilities

• Supervise and coach DSPs to deliver exceptional services.
• Ensure families receive consistent, high-quality care.
• Collaborate with Regional Centers, clients, and behavioral agencies.
• Conduct staff meetings, professional development, and utilization reviews.
• Provide clinical guidance and mentorship.
• Maintain accurate documentation and communication.

Your Role:

What We’re Looking For:

Education & Certifications:

• High School Diploma/GED required; pursuing bachelor’s degree preferred
• Must pass background check/Live Scan
• TB clearance, CPR/First Aid, and Crisis Prevention Certification
