Job summary

Erie Family Health Center in Chicago is seeking a Care Coordinator to support our Social Determinants of Health (SDOH) program. You will work onsite with the clinical team to connect patients to internal and community resources and document needs in Epic.

The role emphasizes screening for needs, enrolling patients in eligible services, educating patients for future SDOH navigation, coordinating transportation and financial assistance, and building partnerships with local social service agencies.

Qualifications

  • High School Diploma or equivalent required.
  • Associates or Bachelor's degree preferred.
  • Minimum two years of work experience in community health or related field preferred.
  • Bilingual English/Spanish preferred.

Responsibilities

  • Screen patients for needs around SDOH and provide linkage to services such as emergency financial assistance, transportation, food assistance, etc.
  • Enroll patients in services, benefits, and programs for which they are eligible.
  • Educate and empower patients to navigate future SDOH needs.
  • Distribute and track SDOH resources and materials.
  • Use Epic to document work, respond to referrals and create SDOH panels.
  • Build effective relationships with local social service agencies and advocate for clients.
  • Performs other duties as assigned.
