Job summary

UCFS Healthcare is seeking an IFP Case Worker to deliver intensive, home-based intervention services to DCF-referred families. The role focuses on preventing out-of-home placement, improving parenting, safety, and case management, and enhancing parent–child relationships.

The position requires 60–70% travel within UCFS’s IFP service area and community collaboration. Strong communication, organization, and a client-centered approach are essential for success.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, or closely related field.
• 3 years’ experience providing home-based services to DCF families including work with diverse populations.

Responsibilities

• Provide outreach and engagement to clients after referral and determine eligibility.
• Assess family strengths and needs to develop a care plan.
• Deliver weekly direct service hours, advocacy, referrals, and goal development with families.
• Participate in monthly joint sessions with Department of Children and Families.
• Maintain up-to-date case records and documentation.

We are currently seeking an IFP Case Worker to provide intensive, home-based intervention services to DCF referred families for purpose of preventing child’s out of home placement, improve parenting, safety, and case management interventions along with improving positive parent – child relationships.

UCFS Hours of Operation: Open 8:00am - 7:00pm; Hours Vary By Day & Location

Expected Schedule: Full-Time, 40 hours/week; Flexible

Outline of Duties and Responsibilities:

• Provide outreach and engagement to client upon referral to the program by conducting an initial phone intake process with the family, collecting basic information and presenting information to team to determine eligibility.
• Complete an assessment of the family including both strengths and areas of need through home visits and various assessments to develop a care plan.
• Provide weekly face-to-face direct service hours to client including support, advocacy, referrals and assistance with the development of positive parenting, safety planning, basic life skills and identified goals in collaboration with the Department of Children and Families in regard to their involvement.
• Participate in monthly in person joint sessions with the Department of Families and the family in regard to the Service Plan established and strengths and challenges within.
• Maintain up-to-date case records on each client family, including weekly contact reports, service agreements, evaluations, PIE records, EPIC system documentation and closing summaries.

Requirements

Minimum Education and Experience

• Bachelor’s in Social Work, Psychology, Counseling, or a closely related field
• 3 years ‘experience providing homebased services to DCF families (including - working with families with complex needs and various degrees of family functioning, working with child and family serving systems, and providing direct services to diverse populations_)

Minimum Certificates, Licenses, Registrations

• A valid driver's license and state mandated levels of auto liability insurance
• Eligibility to and completion of a CPR certification within 30 days of hire
• Eligibility to and completion of Mandated Reporters Training within 30 days of hire

Minimum Skills and Abilities

• Ability for 60-70% travel within UCFS Healthcare’s IFP service area of Windham County and parts of New London County
• Knowledge of the cultural, linguistic, or experiential backgrounds of the communities, children and families to be served
• Proficiency with Microsoft Suite, especially in Outlook
• Ability to provide a strength based, client-centered approach to the delivery of service
• Ability to work in a fast-paced environment and handle multiple tasks simultaneously
• Strong communication and teamwork skills
• Robust interpersonal expertise over digital, in-person, and verbal correspondence
