Job summary

Jobtailor is seeking a patient-focused health support professional to deliver in-home assessments, set up virtual visits, and help patients navigate resources. You will document care in EHRs, coordinate with clinicians, and coach on medication adherence while working independently in patients' homes.

Job description

Build trusted relationships with hard-to-reach and high-needs patients
Set up, support, troubleshoot, and/or conduct virtual primary care and behavioral health visits
Conduct in-home assessments, record vital signs, observe home environments and functional status, and administer screenings
Support behavioral health visits by collecting vitals and administering screenings
Identify and document social needs and navigate patients through social resources and insurance benefits
Reinforce education, coach medication adherence and healthy behaviors, and track care-plan progress
Help patients set goals, track behaviors, and overcome barriers
Follow up on referrals, appointments, and tests; manage delivery or pickup of home testing kits and devices
Deliver supplies and equipment according to internal procedures
Maintain accurate EHR documentation and update the care team
Accompany patients to clinical visits for preventive screening and gap closure
Perform other duties as assigned
Collaborate with physicians, nurses, behavioral health clinicians, community health workers, pharmacists, and urgent care providers

Requirements

Experience in a patient-facing role (e.g., Medical Assistant, Community Health Worker, Health Coach, EMT)
Comfortable working independently in patients’ homes and communities, including with medically or socially complex populations
Strong interpersonal skills and ability to build rapport and communicate clearly with patients and clinicians
Basic clinical and technology proficiency, including vital sign collection, mobile apps, and video calls
Valid unrestricted driver's license and access to an insured vehicle for daily use
Medical Assistant (CMA) or Community Health Worker (CHW) certification, or 2+ years experience in a healthcare setting (preferred)
Experience with value-based care, home-based care, or virtual primary care models (preferred)
Bilingual or multilingual fluency in Spanish or Arabic is a strong plus; additional fluency in Chinese, French, Russian, Tagalog, or Vietnamese is also a plus

Demonstrates expertise in patient engagement and support, including conducting in-home assessments, managing care plans, and navigating social resources. Proficient in virtual primary care and behavioral health visits, with a focus on building relationships and tracking patient progress.
