Job summary

Unison Health in Ohio seeks a management professional to lead the OhioRISE Care Coordination training program, supervise new Care Coordinators, and ensure compliance with state contracts and agency policies. This role blends leadership, training, and direct service as needed.

You will oversee onboarding, certifications, and ongoing coaching, monitor readiness to handle caseloads, report milestones to leadership, and contribute to program improvements while delivering care coordination services

Qualifications

• Bachelor’s or Master’s in Behavioral Health preferred; applicants currently enrolled considered.
• Minimum three (3) years of clinical experience; supervisory and care coordination experience strongly preferred.
• Experience with children and families experiencing SED, trauma, or co-occurring disorders.
• Proficiency with electronic health record systems and Microsoft Office.

Responsibilities

• Participate in hiring process for Care Coordinators and make hiring recommendations.
• Lead onboarding and professional development for new Care Coordinators, ensuring certifications and trainings.
• Ensure training aligns with High Fidelity Wraparound, Trauma-Informed Care standards, and OhioRISE contract requirements.
• Monitor and validate each Care Coordinator’s readiness to independently manage a caseload; provide coaching as needed.
• Report training progress and milestones to leadership and recommend program improvements.
• Provide care coordination services as needed.
