Job summary

Qualifications

• High school graduate or equivalent.
• Two years’ experience with customers/patients, including one year in a health-related field.
• Working knowledge of Microsoft Office programs (Word, Excel, Outlook, PowerPoint).

Responsibilities

• Educates patients/families on the roles of the care management team and ensures timely transitions to the next level of care.
• Assists with Care Management requests: arranging nursing home returns, contacting SNFs/ALFs, and bed availability.
• Arranges resources for durable medical equipment.
• Issues Medicare letters (IMM), MOON notices, ABNs, and HINNs as needed.
• Supports office functions: mail, phones, equipment maintenance, and filing.
• Maintains data in department databases and schedules meetings.
• Contributes to quality and safety initiatives and ensures compliance with policies.
• Maintains punctual attendance and supports department operations.
