Job summary

CareSphere is hiring a Home Care Coordinator to oversee client services, develop care plans, and supervise caregiver assignments in our Suffern, NY office. The role requires strong organization, communication, and the ability to manage multiple tasks in a fast-paced setting.

You'll collaborate with clients, families, and healthcare providers to ensure high-quality in-home care and compliance with regulations, while supporting a collaborative work environment and professional development

Qualifications

• High school diploma or equivalent; associate or bachelor’s in healthcare a plus.
• Experience in home care coordination or related field preferred.
• Strong organizational and attention to detail.
• Ability to work under pressure and meet deadlines.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficiency in Microsoft Office and other relevant software.
• Mon-Fri 8am-4pm in Suffern office.

Responsibilities

• Oversee and coordinate home care services for clients.
• Develop and maintain care plans with clients, families, and providers.
• Schedule and manage caregiver assignments.
• Monitor client progress and adjust care plans as needed.
• Address client and caregiver concerns promptly and professionally.
• Maintain accurate and up-to-date records of care services.
• Ensure compliance with regulations and company policies.
• Work efficiently in a fast-paced environment with multiple tasks.
