Job summary

Penn State Health Holy Spirit Medical Center is seeking an RN Care Manager within the Care Coordination team to lead care planning from admission to discharge. You will collaborate with physicians and a multidisciplinary team to monitor progress and ensure desired outcomes for each patient.

Responsibilities include coordinating care, addressing patient and family needs, and removing barriers to timely, effective care.

Qualifications

• Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
• Two (2) years nursing experience
• AHA BLS certification prior to end of orientation period

Responsibilities

• Works with physician and multidisciplinary team members to develop plan of care for each assigned patient from admission through discharge.
• Ensure patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and evaluations.
• Assess and respond to patient and family needs by coordinating efforts of other team members.
• Identifies and resolves barriers that hinder effective patient care.

Education
