Job summary

Qualifications

• Graduate from an accredited school of nursing with care management/ambulatory care experience preferred
• Current LPN license within the state of practice
• Current BLS certification

Responsibilities

• Coordinate and direct patient care with the care team to ensure policy adherence.
• Facilitate daily care coordination huddles and maintain patient care continuity.
• Manage patient testing, notify providers of results, and relay instructions to patients.
• Follow up after hospital admissions, ER visits, or consultations to ensure proper care.
• Educate patients and families to eliminate barriers to care and promote self-management.
