Job summary

Qualifications

• Graduate of accredited school of Nursing, BSN preferred.
• Current LA RN licensure.
• Working knowledge of alternative payment methodology, insurance coverage, post-acute providers and community resources.
• Demonstrated ability in planning, organizing, leadership, and communication.
• Strong problem-solving skills.
• Current BLS certification through ILCOR approved courses with manikin check-off.
• Two years of Acute Care Hospital, Emergency Department, Case Management or Utilization Management experience preferred.

Responsibilities

• Assess, plan, implement, coordinate and evaluate care to meet patient needs.
• Facilitate progression of care and monitor the plan of care for outcomes and cost-effectiveness.
• Assist with transitions of care and addressing social determinants of health.
• Support patient populations with acute/chronic illness within the health system.
