Job summary

Western AZ Cardiology - Southwest is seeking a Care Coordination Specialist to manage referrals and authorizations for services. You will work with providers and clinic staff to ensure orders are submitted and documentation is sent to referral providers, while keeping patients informed of appointment details and next steps.

The role requires attention to detail, independent work, and strong communication with patients and families, within HIPAA guidelines. Clinic workflows may vary by site.

Qualifications

• Associate degree in medical-related field preferred or technical school in medical-related field.
• 0-1 years experience in a medical or health-related field required.
• 0-1 years experience in referrals/authorizations preferred.
• Proficiency with Google Suite and MS Office; ability to learn Athena and Kronos.

Responsibilities

• Coordinate all incoming and outgoing referrals ordered/received by providers within two business days of submission.
• Ensure orders are completed in the EHR and linked to the referral.
• Validate continuity of care and network availability based on service line.
• Send required documentation to referral providers and update patient records.
• Schedule and notify patients of referral appointments with details (date, time, directions).
• Obtain required authorizations and liaise with insurance to confirm coverage; document in system.
• Maintain HIPAA-compliant handling of patient information.
