Job summary

Northwell Health's Danbury Hospital is seeking a Care Coordination Assistant to facilitate hospital discharges. You will coordinate with a multidisciplinary team, arrange transportation, and ensure accurate transmission of discharge information to post-acute providers.

You will support the discharge planning process, document updates in the electronic medical record, and communicate details to physicians, patients, families, and care partners.

Qualifications

• Minimum high school diploma and two years’ experience in an acute care hospital or equivalent.
• Ability to prioritize and solve problems while working with physicians, patients, families and the care team.
• Excellent proofreading and data entry skills; ability to use a keyboard and enter, retrieve, and transform data.

Responsibilities

• Collaborates with Care Coordination team to prioritize and execute discharges for the day.
• Transmits clinical information to post-acute providers through the electronic referral system.
• Updates discharge plan and informs physicians, patients, families and care team of arrangements.
• Maintains department telephone lines and handles mail and referrals; supports audits and special projects as requested.
