Job summary

Northwell Health is seeking a Clinical Care Coordinator to evaluate and coordinate patient care, discharge planning, and utilization management in an acute-care hospital setting.

Graduates from an accredited nursing program with NYRN licensure are required; BSN preferred with commitment to progress within five years. Minimum two years of medical/surgical experience and strong collaboration with physicians and social work are expected.

Qualifications

• Graduate from an accredited School of Nursing.
• BSN preferred; must enroll in an accredited program within 24 months and obtain BSN within five years of employment date.
• Current license to practice as a Registered Professional Nurse in New York State.

Responsibilities

• Performs concurrent review on all patients and shares problematic cases with Supervisor/Physician Advisor.
• Performs retrospective reviews as required.
• Participates in Utilization Management, Discharge Planning and Case Management statistics.
• Keeps abreast of changes in policies relating to Utilization Management and Case Management.
