Job summary

Qualifications

• Minimum of 1–3 years of clerical experience.
• Experience in a health care setting preferred.
• Strong organizational and communication skills.

Responsibilities

• Attends unit rounds daily to communicate progress of discharge planning.
• Coordinates with case managers and unit staff when rounds are unavailable.
• Initiates referrals to home care, infusion, wound care, and DME providers as needed.
• Verifies start/delivery dates and completes paperwork including authorizations.
• Contacts payers to verify eligibility and coordinates with providers per care team needs.
• Communicates precertification and authorization information to the case manager.
