Job summary

Qualifications

• Requires minimum high school diploma (some college preferred).
• Minimum of two years of experience in an acute care hospital or equivalent.
• Ability to prioritize and problem solve with strong communication skills.
• Proofread and enter data accurately in the EMR and related systems.

Responsibilities

• Collaborate with Care Coordination team to prioritize discharges.
• Transmit clinical information to post-acute providers via electronic referrals.
• Monitor system messages and inform health care team and families of resources.
• Prepare and send required documentation to next level of care.
• Issue Medicare Important Messages as requested.
• Prepare electronic medical records for QIO appeals.
• Coordinate information requests from referral sources to avoid delays and notify of potential denied days.
• Inform physicians, patients, families, and team of discharge arrangements.
• Update community partners on discharge plan changes.
• Document final discharge plan and updates in the EMR; ensure co-signature.
• Manage department mail and triage to correct team member.
• Maintain equipment closet and PAR levels.
• Ensure integrity of the electronic referral system with proper documentation.
• Manage department phone lines to acknowledge receipt of calls.
• Assist director with audits or special projects as requested.
• Fulfill all compliance responsibilities for the position.
• Perform other duties as assigned.
