Job summary

Spectrum Healthcare is seeking a Case Manager to provide comprehensive care coordination and support for individuals in behavioral health treatment. You will help clients access resources, navigate healthcare systems, and advance treatment goals, promoting recovery and improved well-being.

Qualifications

• High school diploma or GED with 2 years of behavioral health experience.
• Bachelor's degree with 1 year of behavioral health experience.
• Bachelor's degree in a human services-related field.
• Completion of a Behavioral Health Technician (BHT) training program.
• Master's degree in a human services-related field (non-licensed).

Responsibilities

• Coordinate care and treatment activities for assigned clients.
• Participate in client staffing and implement treatment plans.
• Provide supportive counseling and document services using the DIAP format.
• Conduct assessments and reassessments under supervision.
• Develop and update treatment plans in compliance with applicable guidelines.
• Collaborate with internal teams, healthcare providers, and community agencies.
• Maintain accurate and timely client records.
• Support clients in navigating healthcare systems and accessing services.
• Ensure compliance with all federal, state, and organizational standards.
• Maintain clinical productivity and participate in team meetings.
• Demonstrate professionalism, cultural competence, and ethical standards.
