Job summary

Qualifications

• Experience in care coordination or discharge planning preferred.

Responsibilities

• Collaborate with the Care Coordination team and support productive patient care processes.
• Manage timely post-acute referrals to aid length of stay and denials mitigation.
• Facilitate communication among interdisciplinary teams, patients, and families.
• Ensure post-acute follow-up appointments to support readmission prevention and continuity of care.
• Deliver required patient notifications and secure signatures for regulatory compliance.
• Coordinate transportation referrals as requested.
