Job summary

Providence Mission Hospital in Mission Viejo, CA is seeking a Care Integration Coordinator for a full-time day-shift role. You will provide essential clerical support to the Care Management Department, coordinating communication among patients, providers, insurers and other stakeholders to ensure safe transitions of care.

Under supervision of licensed clinical staff, you will maintain accurate records, document appropriately, and support regulatory compliance.

Qualifications

• 2 years of Administrative experience, healthcare setting preferred or demonstrated knowledge of healthcare terminology.
• Associate's Degree in Business, Healthcare or related field is preferred.

Responsibilities

• Coordinate communication between patients, health care providers, insurance companies and other stakeholders to ensure safe and efficient transitions of care.
• Maintain accurate records and process documentation to support quality patient care and regulatory compliance.
• Provide essential clerical support to the Care Management Department under supervision of licensed clinical staff.
