Job summary

Providence St. Peter Hospital is seeking a Care Management Coordinator to provide essential clerical support for the Care Management Department.

Under supervision of licensed clinical staff, you will coordinate communication between patients, health care providers, insurance companies and other stakeholders to ensure safe transitions of care and regulatory compliance. You will maintain accurate records, process documentation, and support the delivery of quality patient care.

Qualifications

• 2 years of administrative experience, healthcare setting preferred or demonstrated knowledge of healthcare terminology.
• Associate's Degree in Business, Healthcare, or related field.

Responsibilities
