Qualifications

• Bachelor’s degree in Social Work required.
• 1+ year of clinical social work experience recommended.

Responsibilities

• Provide direct social work services including assessment, treatment, and recommendations to facilitate movement through the continuum of care for patients and families.
• Conduct social work assessments with patient and/or family involvement and identify interventions.
• Collaborate with interdisciplinary team members and advocate for the social work role within care planning.
• Develop and facilitate groups for patients or significant others as appropriate.
• Participate in departmental and unit-based activities and contribute to program development.
