Job summary

Tanner Health System is seeking a Lead Care Coordinator RN to oversee care transitions, coordinate patient care plans, and supervise staff within the Care Coordination team in Georgia. Ideal candidates hold a Georgia RN license with 2+ years of clinical experience, and show professionalism, integrity and collaborative leadership.

Qualifications

• Georgia RN license required; BSN/MSN preferred.
• Two years of related clinical experience; medical–surgical experience preferred.
• Certification or accreditation in Case Management within 18 months of hire.
• Demonstrated professionalism, integrity and ethical behavior.
• Ability to organize, prioritize and make decisions with minimal supervision.

Responsibilities

• Coordinate care for a defined patient population across transitions and services.
• Communicate with care team, families and external providers to plan and monitor transitions.
• Mentor and train new staff; support ongoing team education and performance improvement.
• Act as a liaison with outside agencies and healthcare partners for patient care continuity.
