Job summary

UTMB Health is seeking a Care Coordinator to plan, coordinate, and safely transition acute care patients to the next level of care. You will partner with physicians, nurses, and ancillary services to ensure timely utilization and high-quality outcomes.

This role emphasizes collaboration, patient-centered discharge planning, and seamless communication across care teams, with a focus on safe transitions and service excellence.

Qualifications

• RN with an Associate degree in nursing and Texas license.
• Two years of clinical nursing experience in an acute hospital setting.
• Certification in Care Management CCM or ACM-RN to be obtained within 3 years from date of hire.
• Texas registered nurse required.

Responsibilities

• Coordinate discharge planning with patient, family, and medical team for safe transition to ambulatory setting.
• Collaborate with physicians, nurses, and ancillary services to ensure quality outcomes.
• Lead and mobilize the care team to achieve positive patient outcomes.
• Communicate effectively with physicians, therapists, and other health professionals.
• Develop and communicate a safe, comprehensive transition plan to ambulatory care.
• Uphold service excellence, accountability, and teamwork in daily practice.
