Job summary

The Florida Blue Group is seeking an experienced RN for a hybrid Care Coordinator role focused on navigating members through the continuum of care. You will educate providers on benefits, determine medical necessity, and participate in weekly case reviews to plan discharges and coordinate post-acute services.

Ideal candidates have 2+ years hospital/rehab experience, Florida RN licensure, and familiarity with case management and utilization review. Travel within Florida may be required.

Qualifications

• Bachelor's degree or related equivalent work experience in nursing.
• Registered Nurse licensure in Florida or compact state.
• Experience evaluating benefit determination and level of care in hospital settings.

Responsibilities

• Serve as a point of contact for providers to navigate members through the continuum and educate on benefits.
• Evaluate members in hospital settings for benefit determination and medical necessity.
• Participate in weekly case reviews and coordinate discharge planning.
• Collaborate with Medical Directors, Managers and Case Management for care coordination.
• Travel within the State of Florida occasionally.
