Qualifications

• Minimum 2 years in a medical Care Coordination environment.
• Leadership experience is preferred; supervision of union staff may be required.
• Fluency in Spanish (spoken and written) preferred; valid driver's license.

Responsibilities

• Oversee day-to-day operations of the Care Coordination program.
• Supervise Care Coordination dept. staff including care coordinators and CHWs.
• Ensure scheduling, safety during home visits, and workflow improvements.
• Maintain data entry and grant documentation to meet funder requirements.
• Coordinate staff training and education; communicate with partners and clients.
