Job summary

Aunt Martha’s Health & Wellness is seeking a Care Coordination Outreach Worker to enroll patients in IBCCP, coordinate outreach, and manage follow-up for high- and medium-risk patients. This on-site role at Park Forest, IL requires strong communication and detail-oriented care delivery.

You will support providers, coordinate with community partners, and document activities in the Athena Electronic Health Record. Bilingual English/Spanish is preferred.

Qualifications

• High School Diploma or GED preferred; no college degree required.
• Experience in healthcare, community outreach, patient services, customer service, or care coordination is preferred.
• Bilingual English/Spanish proficiency is preferred.

Responsibilities

• Enroll eligible patients in IBCCP and assist with enrollment.
• Coordinate outreach for active care coordination patients via clinic, home, or hospital visits.
• Schedule follow-up appointments for recently discharged patients and coordinate infant screenings.
• Provide referrals and follow-up support for medium- and high-risk patients.
• Educate patients about screenings, preventive services, and health resources.
• Document patient interactions and outcomes accurately in EHR and program databases.
