Aunt Martha’s Health & Wellness in Park Forest, IL seeks a Care Coordinator Outreach Worker for the Illinois Breast and Cervical Cancer Program (IBCCP). The on-site role focuses on enrolling patients, coordinating follow-up, and connecting patients with preventive services and community resources.

Under the supervision of the Care Coordination Outreach Supervisor, you will manage referrals, document patient interactions, and educate patients on appropriate screenings and healthcare services.

Qualifications

• High School Diploma or GED preferred; no college degree required.
• Experience in healthcare, outreach, patient services, customer service, or care coordination is preferred.
• Bilingual English/Spanish proficiency is preferred.

Responsibilities

• Enroll eligible patients in IBCCP and assist with enrollment.
• Conduct outreach to active patients via clinic, home, or hospital visits.
• Schedule appointments for discharged patients and coordinate medical follow-up.
• Provide referrals and follow-up for medium- and high-risk patients.
