Mercy Gilbert Medical Center is seeking a Care Coordination Nurse to lead discharge planning and care progression for inpatient patients from birth to adulthood. You will coordinate healthcare and social services, balancing patient needs with cost-effective resources and advocating for families throughout transitions of care.

Qualifications

• Two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing in lieu of one year experience
• Graduate of an accredited school of nursing
• Registered Nurse license (AZ or Compact)
• American Heart Association Basic Life Support within 90 days
• Knowledge of managed care and payer environment
• Understand utilization management and case management integration

Responsibilities

• Completes and documents discharge planning assessments.
• Facilitates development of multidisciplinary discharge plans with care team and post-acute providers.
• Oversees and evaluates discharge plan implementation.
• Collaborates with the team to ensure progression of care and appropriate inpatient resource use.
• Coordinates transitions to post-acute services based on patient needs
