CommonSpirit Health is seeking a Care Coordination Manager to provide operational leadership for the Care Coordination department in line with the organization’s model, under the direction of the Director of Care Coordination. You will oversee clinical resource management, progression of care, discharge planning, patient advocacy, and care coordination services.

• BSN required at hire or
• MSW required at hire or if not a registered nurse
• Minimum 3 years of clinical care coordination experience or 3 years of acute nursing leadership with care coordination knowledge
• Operational experience in healthcare with staff development
• License required for MSW

Responsabilidades

• Provide operational leadership for Care Coordination and supervise staff.
• Oversee daily department operations, staffing, workload, scheduling, and resource allocation.
• Ensure timely patient assessments, discharge planning, and care transitions per standards.
• Monitor performance metrics, identify trends and implement improvements.
• Lead multidisciplinary rounds and coordinate with physicians, nursing and UM to ensure care progression.
• Support regulatory readiness and accreditation activities with proper documentation.
